SPECIFIC DEDUCTIBLE The maximum amount of claim dollars paid by the benefit plan for any one covered individual no matter how the great the total claim.
SPECIFIC PREMIUM The premium paid to the stop loss insurance company to cap the claim payment for each covered person at the selected deductible amount.
AGGREGATE DEDUCTIBLE (A.K.A. AGGREGATE ATTACHMENT OR AGGREGATE CAP) The maximum amount of claim dollars paid by the benefit plan in the aggregate. These are all of the claim dollars paid that do not exceed the specific deductible amount per person.
AGGREGATE PREMIUM The premium paid to the stop loss insurance company to cap the total claim dollars paid by the benefit plan.
AGGREGATE ACCOMMODATION The cost of purchasing a monthly accommodation agreement from the stop loss insurance company that caps the total monthly claims paid by the benefit plan at an amount not to exceed the established monthly claims funding amount. Should claims in any one month exceed the monthly funding amount plus any carry forward the stop loss insurance company will advance the difference.
COST CONTAINMENT This fee provides the following services: Pre-certification, Continued Stay Review, Discharge Planning, and Medical Case Management Screening. All of these are necessary for effective cost control.
PLAN CONSULTANT The local sales representative who coordinates all phases of sales and implementation as well as on-going contact with the Plan Administrator.
CLAIMS ADMINISTRATION Variable Protection Administrators, Inc. provides master plan document development, summary plan description books, I.D. cards, on-going eligibility listing, COBRA administration, monthly and year end reporting and of course all phases of claims administration.
