United Employers Corporation is proud to provide its members with the Vision One Exam Plus
Program, which offers a covered in full spectacle examination and discounts on glasses and
contacts through any participating Cole Vision optical outlet.
For over 30 years, Cole Vision has provided quality eyecare and eyewear to patients from coast to coast through its 1600 optical departments in major retailers such as Sears, Montgomery Ward and J.C. Penney. Now, through the Vision One Exam Plus Program, you can make periodic eye examinations and fashionable eyewear a regular part of your personal wellness program.
How Does The Plan Work?
Using your vision plan is easy. Simply visit or call one of the many participating optical providers and identify yourself as a United Employers Corporation member and as a Vision One Exam Plus participant. Using your social security number, the optical outlet will verify you eligibility, your exact plan benefits and any fees which apply. It's as simple as that ... no waiting for reimbursement.
NOTE:If you are getting an eye examination, it is always a good idea to call ahead for an appointment.
Eligibility
All members who are eligible under United Employers Corporation are eligible for the Vision One Plan. All dependents covered under the UEC program are also eligible to receive eyecare benefits through the Vision One Plan.
Basic Benefits
Eligible members and their dependents may receive a covered in full spectacle examination every twelve months. Unlimited pairs of glasses and contacts are available to you and your dependents at significant savings. Eye Examination: A complete examination, refraction and prescription for lenses (Your Cost $O). Contact lens examinations may require additional fees for follow-up visits. Also, if indicated, your doctor may recommend additional procedures (such as dilation) at an additional cost to you.

What If You Choose To Use A Provider That
Is Not In The Cole Vision Network?
Eye Examinations: If you choose to use a provider not in the Cole Vision Network of
providers you are only eligible for partial reimbursement of your eye examination. You
will be reimbursed for the cost of the examination up to a maximum of $30.00.
